Hydro-Québec is subject to the Act respecting Access to documents held by public bodies and the Protection of personal information. The personal information Hydro-Québec collects in this form is necessary for the exercise of its functions under the Hydro-Québec Act. More specifically, the personal information provided enables Hydro-Québec to process your application for its Energy Efficiency Program. The personal information provided will be used exclusively for this purpose and will be accessible only to authorized individuals whose duties require access to this information. If you fail to provide this information, your financial assistance application may be denied.

Before completing this form:

  • Read the program’s Participant’s Guide [PDF 385 KB] carefully.
  • Note that you must fill out a separate form for each eligible heat pump. If you have installed more than one heat pump, use the “Add a heat pump” link in the confirmation of receipt message.
  • Make sure you have all the information you need before you start because you will not be able to save the form and return to it later.
  • Send in only the required documents requested.

Be sure to attach only required supporting documents to your application:

  • Municipal tax bill or the first page of the deed of sale
  • The purchase invoice and the invoice for the system installation, which must also include the AHRI number, model number, brand, installation date, installation address, customer’s name, as well as the name of the contractor installing the system and their Régie du bâtiment du Québec (RBQ) number
  • Photos of the labels containing the model numbers of the indoor and outdoor units

File size

The limit is 5 MB per file and a total of 25 MB for all the files attached to an application. You can send up to five PDF files or photos at a time in one application.

If you are a part-time or seasonal owner-occupant, you must provide your social insurance number so that a Relevé 27 – Paiements du gouvernement [RL-27 slip: government payments] can be issued in your name. Please do not provide your social insurance number with your application.

A Relevé 27 [RL-27 slip] must be produced if the individual, company or partnership to whom it is paying the financial assistance receives it as part of the operations of a company or a property. A landlord is considered to earn property income for any apartment that he does not live in.

If you are a company, a Relevé 27 [RL-27 slip] will be issued in your name.
