Information about your proxy holder
You must enter the name of the person or company you authorize to act on your behalf or to obtain confidential information from your file.
If you wish to authorize:
- a person, please indicate their full name;
- a company, such as a management organization, indicate the company’s name.
Your proxy holder is authorized to act on your behalf in your dealings with Hydro-Québec. If other people have the same powers, please provide a list of your proxy holders.
If they have different powers, you must complete a separate form.
You wish to authorize the proxy holder identified in the proxy form to act on your behalf or to have the following powers:
Manage the account
Your proxy holder may pay and receive your bills, as well as view all information contained in them. Your proxy holder may access your account via their own Customer Space on the Web site and view the information in your account. If you wish, your proxy holder may also receive the original or a copy of your bills.
Make a payment arrangement
Your representative may set an amount and payment dates to settle an unpaid balance in your account.
Your proxy holder may change the rate applied to your account by obtaining consumption data and other information required for analysis.
Obtain or change the phone number and mailing address listed in your file
Your representative may change some personal information in your file.
Notify us of a move
Your representative may change the addresses for which you are responsible for electricity bills.
You are responsible for clearly indicating your choices.
You wish to authorize the designated person identified in the consent form to obtain information about your account or accounts, but not to carry out transactions on your behalf (unlike your proxy holder).

You must enter the account numbers indicated on your bills (the numbers starting with 299).
Duration and signature
You must indicate a date to establish the validity period of this authorization. This date will be entered into your file.
If you do not provide a date, this authorization comes into effect on the date of signature and will be valid for a period of three years. You may request its cancellation, in writing, at any time.
Once you sign this authorization, you relieve Hydro‑Québec of all liability for any inconvenience that may arise from this authorization. This authorization supersedes and voids any prior authorization forms you have signed for the same representative and the same accounts indicated in this form.
